Please read the instructions all the way through before filling out and submitting the form.

This vendor application is a requirement in order to be considered as a vendor at any MomBoss event in 2023.  It does not guarantee you a spot in any event as vendors are rotated. 

Only fill out this form once a year, please do not submit multiple forms for the same business. If you have multiple businesses, then do one form per business (i.e, if you sell scentsy and Tupperware, do a form for each one).

You will not be considered for a spot if there is not a form on file!  No exceptions!

You must have some form of active social media or website for your business.  We will tag or add your business links when advertising.  

Please provide the best email and phone number for communication.  Invoices and other time sensitive info will be sent via email and/or via text.

Once this form is received, you will be emailed a contract that outlines our refund and transfer policies, set up expectations, cancellation policies, etc.
